About The Role Of Population Health Coordinator
The Population Health Coordinator is a pivotal role supporting Kinwell Health’s Quality Improvement, Population Health Strategy, and Health Equity Programs. With the goal of improving our patient’s health and ensuring the organizations meets Hedis measurements, this individual assists in the coordination of health education and awareness campaigns into quality improvement and Managed Care Plan activities. The Population Health Coordinator will work in support of the Population Health team to assist in delivering on Kinwell’s commitment of high-quality, patient-centered care within a medical home framework.
What You’ll Do
Population Health:
- Assist in the coordination of tracking the medical group’s performance metrics.
- Assist in monitoring and reviewing data analytics and electronic health records to monitor and track population health metrics, identifying trends and areas for improvement.
- Outreach to patients to schedule annual wellness visits.
Patient-Centered Medical Home Coordination
- Assist in the coordination of implementing and maintenance of patient-centered medical home principles and practices across all clinical teams.
- Collaborate with healthcare providers to assist in ensuring timely and effective communication, care coordination, and comprehensive patient support.
Quality Improvement And Compliance
- Monitor adherence to quality metrics, regulatory standards, and best practices to ensure compliance with industry regulations and guidelines.
- Participate in audits and assessments to evaluate the effectiveness of patient-centered medical home implementation and population health strategies.
- Other duties as assigned.
What You’ll Bring
- Associate degree, Certified Medical Assistant and (2) years’ work experience in a healthcare setting, with demonstrated experience in care coordination, population health, or patient-centered medical home operations.
- Understanding of data analysis, interpretation, and utilization for decision-making. The ability to successfully manage multiple priorities.
- Prior experience using electronic health records as applicable to area of work (EPIC preferred).
- Bachelor’s degree or equivalent 6 years’ relevant work experience. (Preferred)
- Prior experience as a Clinic or Practice Manager. (Preferred)
- Experience completing a multi-step project from beginning to end independently. (Preferred)
- Prior experience working collaboratively with providers and broader health care community thinking. (Preferred)
- Experience in Primary Care Setting. (Preferred)
- Excellent interpersonal skills including phone communication.
- Skill operating computer in a Windows environment utilizing Outlook, Work, and Excel.
- Excellent verbal and written communication skills.
- Ability to work as a member of a team or independently with minimal supervision.
- Demonstrated ability to exercise judgment to handle sensitive and confidential information in a discrete and professional manner.
- Ability to work in fast paced environment while maintaining strong attention to detail.

The pay for this role will vary based on a range of factors including, but not limited to, a candidate’s geographic location, market conditions, and specific skills and experience.
National Salary Range
$49,700.00 - $74,600.00
